Ganser Syndrome

Ganser syndrome is a rare and unusual dissociative condition whose hallmark feature is the giving of approximate answers, sometimes called vorbeireden, to simple questions, in which the person's response is clearly incorrect but stays in the same general conceptual neighborhood as the correct answer, such as stating that a table has three legs or that two plus two equals five, in a way that suggests the underlying knowledge is present but is being deflected just short of the mark rather than genuinely lost. The condition typically also involves clouding of consciousness, memory gaps, and sometimes perceptual disturbances such as brief hallucinations. The German psychiatrist Sigbert Ganser first described the syndrome in 1898 after observing it in a small group of prison inmates, and it has historically been discussed, and disputed, in relation to prison settings and to the broader question of malingering, since a person consciously faking mental illness for some external benefit could in principle try to produce similarly implausible wrong answers. Most modern clinicians, however, regard genuine Ganser syndrome as an authentic, involuntary dissociative reaction to overwhelming psychological stress, closely related to other dissociative and factitious presentations, rather than a form of deliberate deception, and it is classified within the dissociative disorders in international psychiatric diagnostic systems. The syndrome remains uncommon enough in clinical practice that much of the published literature on it still consists of individual case reports rather than large systematic studies.
