Athazagoraphobia is the fear of forgetting or of being forgotten, including forgetting people, places or things important to oneself. It can appear as a fear of one's own memory failing, as with an aging relative, or as a fear of being abandoned or erased from other people's memory. The condition is an anxiety response to the prospect of memory failure, not a memory disorder itself.
